Cassandra R. Lin,
MSN, APRN, FNP-BC, FMACP
Cassie is a certified family and functional medicine nurse practitioner with a wealth of experience spanning over 15 years in the medical field. She began her career with a bachelor’s degree in nursing and spent a decade as a registered nurse specializing in women’s health and labor and delivery. Driven by a passion for advancing her expertise, Cassie pursued her master’s in nursing and transitioned to providing patient care as a family nurse practitioner. Her unyielding passion for helping others shines through in every patient interaction. Known for her warm demeanor and compassionate approach, Cassie genuinely cares for and respects her patients, making them feel valued and understood. As her family prepares to retire from the military and transition to civilian life in Tipp City, she aims to build a practice that not only serves the needs of local residents but also reflects her long-term commitment to the area.
Being a military wife, Cassie embraced the diverse life and work experience offered by each military relocation. This allowed her to gain valuable insights working in various hospitals and clinics across the States. The vast experiences helped her identify gaps in the medical community and the care patients receive. This unique journey fueled her passion and led to the launch of Living Well Functionally, an initiative aimed at addressing the challenges patients face in receiving care by providing integrative medical care. In this venture, she strives to bridge the divide between traditional medicine and what patients want by ensuring that every patient feels supported and receives the attention they deserve.
As a mother, Cassie understands the challenges and complexities of navigating healthcare for her family. She has faced the pressures of a rigid medical system that often overlooks the individual needs of patients, leaving them feeling voiceless and unvalued. Her experiences have fueled her passion for providing a different kind of care—one that is centered around personalization and empathy. Cassie believes that every patient deserves to be heard, and her commitment lies in ensuring that each interaction is tailored to meet their unique needs. By focusing on individual care rather than a one-size-fits-all approach, she strives to create a healthcare experience that fosters trust, respect, and a genuine sense of value for every patient she encounters.
Outside of her professional life, Cassie finds immense joy in spending quality time with her husband and three children. Together, they embark on various DIY home renovation projects, transforming their living space into a true family haven. Gardening is another passion of hers, where she loves to work alongside her family, planting flowers and growing organic vegetables. Cassie's love for animals is evident in her care for their chickens and her affectionate cats.

Cindy M. Fleming
MSN, APRN, FNP-BC
Cindy is a graduate of Chamberlain University, where she earned her Master’s degree in Nursing Science with a specialization as a Family Nurse Practitioner, becoming board-certified through the American Nurses Credentialing Center (ANCC). Currently, she is pursuing certification as a functional medicine provider with a focus on molecular biology through the Institute of Functional Medicine, where she is engaged in an intensive two-year study program aimed at understanding the root causes of disease and dysfunction to promote optimal health and well-being.
With 25 years of experience in the medical field, Cindy began her career in bedside nursing after earning her degree from Edison State College. Her diverse experience includes surgical nursing, physical rehabilitation, telemetry, and three years as an acute and chronic dialysis nurse, where she managed anemia and provided various treatments both in and out of the hospital. She also spent two years in quality improvement, collaborating with acute care providers and hospitalists to analyze patient cases and enhance outcomes through improved patient safety protocols and practices.
Cindy’s commitment to quality care led her to a five-year tenure as a primary care provider, followed by two years as a gastroenterology specialist. Throughout her career, her primary goal has been to offer exceptional care, guidance, and advice to her patients, integrating her extensive knowledge and expertise to deliver holistic, evidence-based, and professional healthcare.
Outside of her work as a functional medicine primary care provider, Cindy is a devoted mother of two teenagers and two young adult children, and is excited to be expecting a grandchild. She and her husband live on a mini farm with two poodles (one standard, one teacup), a cat, goats, chickens, and turkeys. They also host an annual youth ministry camp-out on their property.

Dr. Adam Welch is double board-certified in Family Medicine and Emergency Medicine. He is passionate about patient advocacy, focusing on promoting lifestyle changes, empowering patients to make informed decisions, and providing holistic care that views individuals as more than just the sum of their parts. Outside of his practice, Dr. Welch enjoys traveling, hunting, and high-energy activities such as skiing.

He oversees key administrative functions, from scheduling, patient outreach, marketing to billing processes. With a strong focus on fostering team collaboration, he is committed to creating a supportive environment where everyone can thrive and contribute towards delivering quality healthcare to the community they serve.
